A heated dispute has broken out among the most powerful players in the ph=
armaceutical industry =E2=80=94including drug makers, pharmacists, and PB=
Ms=E2=80=94with many blaming each other for the rising price of medicine.=
With billions in profit on the line, the pharmaceutical and health produ=
cts industry has already spent $78 million on lobbying in the first quart=
er of this year, a 14 percent jump over last year, according to the Cente=
r for Responsive Politics. One area of contention is between PBMs and pha=
rmacists. When independent pharmacists visited Capitol Hill in late April=
, they came with a brochure depicting benefit managers as sharp-toothed d=
ogs, grabbing bags of money. Rep. Lloyd Doggett (D-Texas) says the indust=
ry war is in some ways a positive sign. "We have moved from 'There is no =
problem' to 'It's not my fault,'" he says. "It begins to focus attention =
on what so many of my constituents already know the problem is, which is =
price gouging." No single proposal has emerged as a clear winner in the b=
id to lower prices. Members of Congress have put forward a grab-bag of op=
tions, each of which would help or hurt different industry players.

AstraZeneca has launched a recall of one lot of professional (physician) =
sample bottles containing eight tablets of ticagrelor (Brilinta) 90 mg ta=
blets. The voluntary recall follows a report that a professional sample b=
ottle containing eight tablets of ticagrelor 90 mg also contained another=
medicine, lesinurad (Zurampic) 200 mg, also made by AstraZeneca. The mov=
e is a precautionary measure=E2=80=94AstraZeneca has not received any rep=
orts of adverse events related to this recall. The affected lot, #JB5047,=
was distributed to U.S. physicians between March and April this year. Ot=
her forms and dosage strengths of ticagrelor are not affected by the volu=
ntary recall, nor does the recall affect lesinurad.

Lupin Pharmaceuticals has launched a recall of one lot of norethindrone a=
cetate and ethinyl estradiol 1 mg/0.02 mg chewable and ferrous fumarate 7=
5 mg (Mibelas 24 Fe) tablets at the consumer level. The product, an oral =
contraceptive, is being recalled due to a confirmed market complaint abou=
t a packaging error in which the blister was rotated 180 degrees within t=
he wallet, reversing the weekly tablet orientation and making the lot num=
ber and expiration date no longer visible. As a result, the first 4 days =
of therapy would have had four non-hormonal placebo tablets instead of th=
e active tablets. The lot being recalled is L600518, with an expiration d=
ate of May 2018.

QuintilesIMS Institute has issued a report that examines challenges healt=
h systems need to overcome in order to maximize the benefits that precisi=
on medicines offer to patients. "The pharmacology of precision medicines =
is complex. Pharmacists have an important role to play in understanding t=
hese medicines, interpreting diagnostic results, and ensuring that the re=
quisite testing has been and is being done so that the right patients are=
treated," notes Murray Aitken, executive director of Quintiles IMS Insti=
tute and coauthor of the report. Aitken says about 45 percent of today's =
roughly 80 precision medicines come to market with black box warnings ind=
icating evidence of an association with serious risk. As a result, pharma=
cist engagement with patients is important. "Pharmacists can enroll patie=
nts on pharmacovigilance programs," he said, noting that the relationship=
patients form with pharmacists can help any serious adverse effects be i=
dentified more quickly. The report also addresses such concerns as clinic=
al decision-making, shortcomings in health system diagnostics, patient pr=
ivacy issues, global availability, gathering big data, and cost.

An analysis of government data by the Wall Street Journal found expensive=
medicines are increasingly raising costs for older adults and other bene=
ficiaries in Medicare Part D, despite drugmakers' increasing discounts an=
d federal legislation meant to reduce out-of-pocket costs. The median out=
-of-pocket cost for a drug purchased through Part D was $117 in 2015, up =
from $79 in 2011, in inflation-adjusted dollars, the analysis found. Fact=
ors driving the trend include sharply rising drug prices, which grew by a=
n average 14 percent a year from 2011 to 2015, and the introduction of ne=
w medicines with prices that commonly exceed $50,000 annually. In additio=
n, the complicated design of Part D requires patients to pay a percentage=
of their drugs' total retail price, a particular burden for those who us=
e expensive medicines. Medicare patients taking expensive drugs do not be=
nefit directly from the rebates that pharmaceutical companies often give =
to insurers.

Investigators at The Scripps Research Institute (TSRI) have found a way t=
o structurally modify vancomycin to make an already-potent version of the=
antibiotic even stronger. "Doctors could use this modified form of vanco=
mycin without fear of resistance emerging," said Dale Boger, cochair of T=
SRI's Department of Chemistry, whose team announced the finding in the Pr=
oceedings of the National Academy of Sciences. The TSRI researchers had p=
reviously shown it is possible to add two modifications to vancomycin to =
make it even more potent. "With these modifications, you need less of the=
drug to have the same effect," Boger said. In the new study, the researc=
hers made a third modification that, combined with the previous modificat=
ions, gives the drug a 1,000-fold increase in activity. When tested again=
st Enterococci bacteria, the new version of vancomycin killed both vancom=
ycin-resistant Enterococci and the original forms of Enterococci. The nex=
t step is to come up with a way to synthesize the modified vancomycin usi=
ng fewer steps in the lab, because the current method takes 30 steps.

Gov. Rick Snyder of Michigan signed on Thursday legislation authorizing t=
he state Department of Health and Human Services to issue a standing orde=
r pre-authorizing the distribution of naloxone by pharmacists. "Naloxone =
is a tool in the fight against opioid addiction that can save lives immed=
iately, and we need to make sure all residents statewide have access, bot=
h in rural areas and urban centers," Snyder said. Naloxone is now availab=
le OTC in Michigan; however, pharmacists must register with the state in =
order to participate. Registration for pharmacies began on Thursday.

Suspected Drug Thefts Persist at VA Centers

Federal authorities are investigating dozens of new cases of possible opi=
oid and other drug theft by employees at Veterans Affairs (VA) hospitals.=
Thirty-six criminal investigations have been opened by the VA inspector =
general's office from October 1 through May 19, bringing the total number=
of open criminal cases to 108 involving theft or unauthorized drug use. =
Doctors, nurses, or pharmacy staff in the VA's network of more than 160 m=
edical centers and 1,000 clinics are suspected of siphoning away controll=
ed substances for their own use or street sale. The rate of reported miss=
ing drugs at VA health facilities is more than double that of the private=
sector. Drug Enforcement Agency investigators cite in part a larger quan=
tity of drugs kept in stock at the bigger VA medical centers to treat a h=
igher volume of patients, both outpatient and inpatient, and for distribu=
tion of prescriptions by mail. Michael Glavin, an information technology =
specialist at the VA, says he has heard numerous employee complaints of f=
aulty VA technical systems that track drug inventories, leading to errors=
and months of delays in identifying when drugs go missing.

llion fewer than the estimate for the House's previous bill. The new scor=
e, which reflects last-minute revisions that Republicans made to pass the=
measure, calculates that the American Health Care Act would reduce the f=
ederal deficit by $119 billion between 2017 and 2026. That represents a s=
maller reduction than the $150 billion CBO estimated in late March, large=
ly because House leaders provided more money in their final bill to offse=
t costs for consumers with expensive medical conditions and included lang=
uage that could translate to greater federal spending on health insurance=
subsidies.

a multifaceted effort to restrain high prescription-drug prices, focusing=
on speeding cheaper generic medicines onto the U.S. market. "Simply put,=
too many patients are priced out of the medicines they need," Gottlieb t=
old a congressional subcommittee Thursday. "While FDA does not have a dir=
ect role in drug pricing, we can take steps to facilitate entry of lower-=
cost alternatives to the market, and increase competition." Gottlieb plan=
s to have FDA intervene in cases in which brand-name drug companies are a=
ccused of using agency safety rules to refuse to make drugs available to =
generic manufacturers. "FDA has an important role to play," he said, "in =
making sure that its statutory and regulatory processes are working as in=
tended, and not being manipulated in ways that FDA and Congress did not i=
ntend." Part of the commissioner's drug-price initiative will be an effor=
t to accelerate generic-drug approvals by reducing the backlog of applica=
tions at the agency.

Maryland Enacts Law Barring 'Unconscionable' Drug-Price Increases

On Friday, Maryland enacted a law that prohibits steep price hikes for ge=
neric drugs. Gov. Larry Hogan allowed the measure to become law but withh=
eld his signature due to reservations about the legislation's effect and =
its legal footing, according to a letter Hogan wrote to the speaker of th=
e state House. "I am not convinced that this legislation is truly a solut=
ion to ensuring Marylanders have access to essential prescription drugs, =
and may even have the unintended consequence of harming citizens by restr=
icting their access to these drugs," Hogan wrote. Under the law, "unconsc=
ionable" price increases for generic drugs are prohibited and the state a=
ttorney general can sue drug makers to try to roll back some price hikes.=
The Association for Accessible Medicines, meanwhile, said the new law wa=
s unconstitutional and "will have the unintended consequence of driving a=
way generic manufacturers" from Maryland.

A heated dispute has broken out among the most powerful players in the ph=
armaceutical industry =E2=80=94including drug makers, pharmacists, and PB=
Ms=E2=80=94with many blaming each other for the rising price of medicine.=
With billions in profit on the line, the pharmaceutical and health produ=
cts industry has already spent $78 million on lobbying in the first quart=
er of this year, a 14 percent jump over last year, according to the Cente=
r for Responsive Politics. One area of contention is between PBMs and pha=
rmacists. When independent pharmacists visited Capitol Hill in late April=
, they came with a brochure depicting benefit managers as sharp-toothed d=
ogs, grabbing bags of money. Rep. Lloyd Doggett (D-Texas) says the indust=
ry war is in some ways a positive sign. "We have moved from 'There is no =
problem' to 'It's not my fault,'" he says. "It begins to focus attention =
on what so many of my constituents already know the problem is, which is =
price gouging." No single proposal has emerged as a clear winner in the b=
id to lower prices. Members of Congress have put forward a grab-bag of op=
tions, each of which would help or hurt different industry players.

sample bottles containing eight tablets of ticagrelor (Brilinta) 90 mg ta=
blets. The voluntary recall follows a report that a professional sample b=
ottle containing eight tablets of ticagrelor 90 mg also contained another=
medicine, lesinurad (Zurampic) 200 mg, also made by AstraZeneca. The mov=
e is a precautionary measure=E2=80=94AstraZeneca has not received any rep=
orts of adverse events related to this recall. The affected lot, #JB5047,=
was distributed to U.S. physicians between March and April this year. Ot=
her forms and dosage strengths of ticagrelor are not affected by the volu=
ntary recall, nor does the recall affect lesinurad.

Lupin Pharmaceuticals has launched a recall of one lot of norethindrone a=
cetate and ethinyl estradiol 1 mg/0.02 mg chewable and ferrous fumarate 7=
5 mg (Mibelas 24 Fe) tablets at the consumer level. The product, an oral =
contraceptive, is being recalled due to a confirmed market complaint abou=
t a packaging error in which the blister was rotated 180 degrees within t=
he wallet, reversing the weekly tablet orientation and making the lot num=
ber and expiration date no longer visible. As a result, the first 4 days =
of therapy would have had four non-hormonal placebo tablets instead of th=
e active tablets. The lot being recalled is L600518, with an expiration d=
ate of May 2018.

QuintilesIMS Institute has issued a report that examines challenges healt=
h systems need to overcome in order to maximize the benefits that precisi=
on medicines offer to patients. "The pharmacology of precision medicines =
is complex. Pharmacists have an important role to play in understanding t=
hese medicines, interpreting diagnostic results, and ensuring that the re=
quisite testing has been and is being done so that the right patients are=
treated," notes Murray Aitken, executive director of Quintiles IMS Insti=
tute and coauthor of the report. Aitken says about 45 percent of today's =
roughly 80 precision medicines come to market with black box warnings ind=
icating evidence of an association with serious risk. As a result, pharma=
cist engagement with patients is important. "Pharmacists can enroll patie=
nts on pharmacovigilance programs," he said, noting that the relationship=
patients form with pharmacists can help any serious adverse effects be i=
dentified more quickly. The report also addresses such concerns as clinic=
al decision-making, shortcomings in health system diagnostics, patient pr=
ivacy issues, global availability, gathering big data, and cost.

medicines are increasingly raising costs for older adults and other bene=
ficiaries in Medicare Part D, despite drugmakers' increasing discounts an=
d federal legislation meant to reduce out-of-pocket costs. The median out=
-of-pocket cost for a drug purchased through Part D was $117 in 2015, up =
from $79 in 2011, in inflation-adjusted dollars, the analysis found. Fact=
ors driving the trend include sharply rising drug prices, which grew by a=
n average 14 percent a year from 2011 to 2015, and the introduction of ne=
w medicines with prices that commonly exceed $50,000 annually. In additio=
n, the complicated design of Part D requires patients to pay a percentage=
of their drugs' total retail price, a particular burden for those who us=
e expensive medicines. Medicare patients taking expensive drugs do not be=
nefit directly from the rebates that pharmaceutical companies often give =
to insurers.

Investigators at The Scripps Research Institute (TSRI) have found a way t=
o structurally modify vancomycin to make an already-potent version of the=
antibiotic even stronger. "Doctors could use this modified form of vanco=
mycin without fear of resistance emerging," said Dale Boger, cochair of T=
SRI's Department of Chemistry, whose team announced the finding in the Pr=
oceedings of the National Academy of Sciences. The TSRI researchers had p=
reviously shown it is possible to add two modifications to vancomycin to =
make it even more potent. "With these modifications, you need less of the=
drug to have the same effect," Boger said. In the new study, the researc=
hers made a third modification that, combined with the previous modificat=
ions, gives the drug a 1,000-fold increase in activity. When tested again=
st Enterococci bacteria, the new version of vancomycin killed both vancom=
ycin-resistant Enterococci and the original forms of Enterococci. The nex=
t step is to come up with a way to synthesize the modified vancomycin usi=
ng fewer steps in the lab, because the current method takes 30 steps.

r pre-authorizing the distribution of naloxone by pharmacists. "Naloxone =
